The Faculty/Staff Health & Safety Committee initiated an Ergonomics Assessment program several years ago. Overall, the program has been very well received and many employees have benefitted from it. Responsibility for this program has now been turned over to Human Resources. The program provides an introductory ergonomics assessment, which includes a visit from a Certified Occupational Therapist (OT) or Kinesiologist and an follow-up report with recommendations. Occupational Concepts Ltd. of Moncton has been engaged to complete the ergonomic assessments. The follow-up report is sent to the committee and is then forwarded to the employee, the employee's supervisor or department head and the appropriate Dean or Director. Any resulting action then becomes the responsibility of the employee to initiate with his or her supervisor through the appropriate University channels. (The health & safety committee does not purchase any equipment, furniture or services resulting from the therapist's recommendations.) The cost of the assessment is covered by Human Resources. While the budget available for this program is limited, the committee is normally able to accommodate most requests. The ergonomic assessments are coordinated by Lori Geldart. Lori serves as the liaison between HR, the Committee, the employee and the occupational therapist. she can be contacted by email at hr@mta.ca or by phone at 2280. To request an ergonomic assessment, simply complete the two forms below and forward them to Lori in HR. (The assessment cannot be scheduled without the completed forms.) Lori will then contact you to coordinate a date and time for the assessment. For
