Exam Accommodations

If you require exam accommodations, you must come in to meet with the coordinator.

For each course for which you require accommodations, you must obtain a Request for Accommodated Exam Form authorized by the Coordinator.

This form must then be signed by the Professor/Instructor acknowledging that he/she is aware that the student will write with accommodations. The professor/Instructor will keep a copy and a copy is returned to the Wellness Center. Exam accommodations will not be provided if this form has not been returned to the Coordinator.

    • Midterms/Test requests must be received 5 business days prior to the test date.
    • Finals Exam requests must be received by the office 2 weeks prior to the exam date.

Note Taking

If you require the services of a note taker you must complete the Request for Note Takers Form.

  • Please register as soon as possible to ensure that you receive notes from the beginning of the semester.
  • Notes will be placed in a personal file located at the Wellness Center or emailed directly to you.
  • Pick up your notes on a weekly basis. If you do not pick up notes on a regular basis, note taking services may be discontinued.
  • You are responsible to notify the coordinator if there are concerns with your notes i.e. not on time each week, are not comprehensive,  are simply notes from Moodle or notes are missing. These issues will not be remedied at the end of the term.
  • You are responsible to notify the coordinator immediately if you drop a course so that note taking may be discontinued.

Tutors

Student Life creates a list of available tutors each year. Students can access this list at any time if they require a tutor. Disability Services will coordinate tutors based on individual needs and learning styles upon request by a student registered with Disability Services. Typically, tutors will be paid using money a student has received under the Grants for Students with Permanent Disabilities.
