If you would prefer to submit an anonymous report, you may download the pdf version and drop or send it via campus mail to SHARE, Student Health Centre, 15 Salem Street.

This form enables all members of the Mount A Community to report incidents or crimes sexual harassment, sexual assault, discrimination, hate crimes, assault and other violations of University Policies or criminal acts. Anyone can report by completing the form below. SHARE will review forms sent electronically and collaborate with Campus Security and the Dean of Students to determine a best response to each report.

Confidential Reporting is not intended for emergencies. Reports of emergencies should always be directed to RCMP by calling 911, or Campus Security at 364-2228 or 364-2452
This is a serious incident-reporting service, and all reports will be investigated.
Please do not report issues and disputes related to roommates or residence life. These should be addressed with assistance from your Don or RA.
