Registration Forms

Here are the Web versions of three forms:

  1. Conference registration and fees payment.
  2. Booking for on-campus accommodation and meals.
  3. 1998–99 STLHE membership application, if you are not already a Society member and wish to join.
Please complete the first form and the others as required, using the Print command on your Web browser to print the forms for completion. Although Web page formatting is imprecise, you should find that a 12pt font setting in Netscape, or the "medium" font setting in MS Explorer, produces good printed results.

Note that the mailing deadline for early registration is

Friday, 1 May, 1998.
