Accidental death & dismemberment insurance (SSQ)
This voluntary plan provides accidental death, dismemberment and loss of use insurance for eligible employees and, if desired,for spouses and dependent children. This insurance coverage will be effective on the date the completed application form is received by Human Resources.
- Insurance coverage ranges from a minimum of $10,000 to a maximum of $300,000
- The monthly cost for the employee only plan is $0.14 for each $10,000 of coverage
- The monthly cost for the employee and family plan is $0.24 for each $10,000 of coverage
