Critical Illness Insurance (SSQ)
Critical Illness insurance is a voluntary benefit designed to provide a lump sum payment in the event that the covered individual is diagnosed for the first time with a given covered Critical Illness while the insurance is in force. It can provide the funds to and means to preserve your quality of life, protect personal assets and allow the freedom and flexibility to choose the kind of health care you want.
MtA benefit eligible employees, their spouse, and dependent children are eligible for coverage. Dependent children are covered up to 21 years of age, unless attending an institution of Higher Learning on a full-time basis, in which case they will be covered until 25 years of age. To apply complete the enrollment form, found in the link to the right, and return it to Human Resources. Coverage will be effective on the first day of the month coinciding with or next following your date of employment. The premium is paid for by the employee.
- The first $50,000 of coverage for you and your spouse and $25,000 for dependent children is guaranteed.
- You have the option to purchase up to $150,000 in coverage for both you and/or your spouse. Coverage is limited to a $25,000 maximum for dependent children.
- Any amount in excess of $50,000 is subject to medical evidence and approval by the carrier. If denied, you are covered for the $50,000 guaranteed amount.
